Research shows that sleep apnea and hearing loss are linked. But the connection between hearing and sleep goes further: hearing loss can disrupt sleep quality on its own. Here's what the studies show on both.

Most people think of hearing loss and sleep issues as two separate problems. One is about the ears. The other is about the brain winding down at night. Research suggests they're more connected than many people think. Sleep apnea is one of the clearest indicators that hearing and sleep aren't separate systems — what happens to one affects the other, and, as it turns out, the reverse is also true.
At least 25 million U.S. adults have sleep apnea, and most cases go undiagnosed. It's marked by repeated pauses in breathing during sleep, sometimes hundreds of times a night, each one accompanied by a temporary drop in blood oxygen.
A 2016 study¹ following more than 16,000 participants found that people with sleep apnea had 26% higher odds of high-frequency hearing loss, 127% higher odds of low-frequency hearing loss, and 29% higher odds of loss across both ranges. A separate study² that same year found the risk climbed further among people with severe apnea and low blood oxygen levels during sleep.
Researchers haven't established that apnea directly causes hearing damage, but they’ve proposed two mechanisms that could explain the pattern:
None of this means sleep apnea guarantees hearing loss, or that CPAP treatment will reverse hearing loss that's already occurred. But the association is consistent enough across study populations that anyone diagnosed with sleep apnea has a reasonable, evidence-backed reason to get a baseline hearing test rather than waiting for symptoms to show up on their own.
So far, the causal arrow points one direction: apnea toward hearing loss. But there's a second, separate line of research asking whether the arrow also runs the other way — whether hearing loss disrupts sleep patterns. Again, the research shows reasonable evidence that the answer is yes.
For people with healthy hearing, understanding speech in everyday noise happens with little conscious effort. The ears and brain work together to focus on the voice that matters while filtering out competing sounds.
For people with sensorineural hearing loss, the most common form of age-related hearing loss, the ear sends fewer and weaker signals to the brain. As a result, the brain works harder to fill in the gaps and follow conversations, especially in noisy places. This extra listening effort leads to mental fatigue and increased stress—a known precursor for poor sleep quality.

One of the body's primary stress hormones is cortisol. Under normal circumstances, cortisol follows a predictable daily rhythm: it's highest in the morning to help you wake up and gradually declines throughout the day so that levels are low by bedtime.
Chronic stress can disrupt that rhythm. Instead of falling in the evening, cortisol may remain elevated, keeping the body in a more alert, activated state when it's supposed to be preparing for sleep. Researchers have found that people with chronic insomnia often have higher cortisol levels than people who sleep well³. Many people with insomnia also experience an exaggerated surge in cortisol shortly after waking. In other words, elevated cortisol and poor sleep are strongly linked.
Given the connection between stress and sleep, the next question is whether hearing loss itself raises cortisol levels. Multiple studies suggest it does.
One study of older adults⁴ found that those with moderate to severe hearing loss showed significantly altered cortisol activity compared to those with normal or mild hearing loss, with the effect growing stronger as high-frequency hearing declined. Meanwhile, another study observing children⁵ found that children with hearing loss had a bigger cortisol spike shortly after waking than children with normal hearing (the same pattern seen in people with insomnia).
Research on tinnitus points in the same direction. One study measured cortisol in hair samples, which reflects stress levels over several months. People who perceived their tinnitus as more severe also had higher cortisol levels.
Another 2025 study⁶ had tinnitus patients complete a demanding listening task and measured a stress marker in their saliva before and after. The stress marker rose significantly right after the task, showing that a single stretch of hard listening can trigger a real stress response in the body.
In short: elevated cortisol is known to disrupt sleep, and a growing body of research shows that hearing loss and tinnitus are both associated with elevated cortisol.
Research suggests poor sleep itself can also make the body's stress response more reactive, meaning smaller stressors may produce larger increases in cortisol. This can reinforce a self-perpetuating cycle of poor sleep:
